A Patient is referred to CHAN by a provider such as a physician, DSS, Health Department, Emergency Room, Good Samaritan Clinic, or other health providers. Applicants may also come directly to the CHAN office and will be screened for eligibility. The patient applies for services through an Income Eligibility Specialist located in the CHAN office. The screening includes eligibility for Medicaid, Food Stamps, WIC and other services. The applicant is connected to all services that he or she might be eligible for. The CHAN staff also encourages the applicant to register to vote and will facilitate the process.CHAN office is located at: 538 Scotts Creek Road, Annex A in Sylva, North Carolina.
CHAN patients are encouraged to become responsible for their own health and are asked to comply with certain aspects of their health care management.
Patients receive an Identification card to present at the physician's office, pharmacies, emergency room and other approved providers at each visit.
The CHAN office schedules appointments for patients and tracks diagnosis, treatment, and medications after each visit. Patients are responsible for keeping each appointment and for ensuring that they have transportation. Patients are responsible for taking medications as directed. This is an important part of "getting better". Patients also agree to follow the instructions of their physician.
Patients re-enroll every six months to ensure that the financial and residential eligibility is still in effect.
