WHO IS ELIGIBLE FOR CHAN?

Community Health Access Network assists Jackson county residents who are

• uninsured,
• low income (below 150% of poverty) and
• between the ages of 18 and 64 who are
• ineligible for Medicaid and who cannot afford physician fees.

The primary goal of CHAN is to develop and manage a coordinated system for helping uninsured people who cannot afford medical care, or access to physician services.
